We now have an on-site physiotherapist who can deal with new musculoskeletal pain. Appointments are available at Comberton surgery on Tuesdays and these can be booked via reception. More information is available via this short video

PCN Care Co-ordinator By Referral

Care Coordinators mainly complete admin support for the surgeries however, but can assist patients with the following:

  • Supporting patients with long term medical conditions
  • Support for patients after hospital discharge
  • Help patients to coordinate their medical appointments
  • Book patients into support clinics i.e. Alzheimer’s clinic/Dementia hub
  • Assist with wheelchair referrals
  • Supporting patients with complex needs
  • Connecting Patients to other services in the community
