Registration

To register with the practice, please print and COMPLETE BOTH of the following forms and bring them to the surgery. If you prefer, you can post the forms or email to admin.combertonsurgery@nhs.net.

 For new patients aged under-16, please complete the form above (GMS1) as well as the following form.

If you are living in the practice area for 3 months or less, you can register as a temporary patient. This means you continue to be registered with your home practice but can access service from this practice while you’re in the area. After 3 months your registration will expire and you will be asked to register as a permanent patient if you’re still living here. To register as a temporary patient, please print and complete the following form and bring to the surgery.

Change of name or address

If you have changed your name, contact details, or have moved to a new address within the practice catchment area, please complete the following form.

We would like to apologize to all our patients for the unacceptably slow service from the dispensary. We are aware that we have been unable to fulfil the turnaround times for repeat prescriptions that we had promised, and we are also aware that many patients have had to queue for long periods of time to collect their medication.

We have been facing unprecedented demand at a time when we are looking to recruit a new dispensary manager and when we have had high levels of staff sick leave.

Please be assured that we are taking active steps to improve the dispensary and we hope to be able to provide a better service in the New Year.
